National Perinatal Loss Collaborative
A Jane’s Room and Return to Zero: HOPE Initiative
Effective Date: January 1, 2026

1. Purpose
This Member Agreement and Code of Conduct ("Agreement") outlines the expectations, responsibilities, and guidelines for participation in the National Perinatal Loss Collaborative ("Collaborative"). By joining and using the platform, you agree to uphold these standards.
The Collaborative exists to support clinicians and organizations in delivering compassionate, evidence-based care to families experiencing pregnancy and infant loss. Our community is built on trust, professionalism, and shared learning.

2. Membership Eligibility and Use
Membership is intended for healthcare professionals, administrators, and affiliated organizations engaged in perinatal care and bereavement support.
Members agree to:
• Provide accurate and current information
• Maintain the confidentiality of login credentials
• Use the platform for professional, educational, and collaborative purposes only

3. Professional Conduct Expectations
All members are expected to:
• Engage respectfully and professionally at all times
• Contribute constructively to discussions and shared learning
• Honor diverse perspectives, disciplines, and experiences
• Communicate with empathy, recognizing the sensitive nature of perinatal loss

4. Confidentiality and Patient Privacy
Protecting patient privacy is essential.
Members agree to:
• Never share protected health information (PHI) or identifiable patient details
• De-identify all case discussions in accordance with HIPAA and professional standards
• Avoid sharing any information that could reasonably identify a patient, family, or colleague
Violation of these standards may result in immediate removal from the platform.

5. Community Participation Guidelines
To maintain a safe and supportive environment:
Members must not:
• Share harmful, discriminatory, or inappropriate content
• Engage in harassment, bullying, or intimidation
• Promote personal services, products, or solicit business without permission
• Share misleading or non-evidence-based clinical information
Members are encouraged to:
• Ask questions and seek guidance
• Share best practices and lessons learned
• Support peers with thoughtful insight and experience

6. Use of Content and Intellectual Property
All materials provided through the Collaborative—including courses, toolkits, webinars, and resources—are the intellectual property of the Collaborative or its partners.
Members agree:
• Not to reproduce, distribute, or share materials outside their organization without permission
• To use materials for internal education and care improvement only
• To appropriately credit sources when referencing Collaborative content

7. Continuing Education (CE) Participation
For members participating in CE offerings:
• Completion records will be maintained for accreditation purposes
• Members are responsible for accurately completing required coursework
• Misrepresentation of participation or completion may result in revocation of CE credit

8. Organizational Membership Responsibilities
For Organizational, Regional, and System Members:
• Designated administrators are responsible for managing user access
• Members within the organization must adhere to this Agreement
• Organizational data shared by the Collaborative will be aggregated and de-identified

9. Platform Integrity and Security
Members agree not to:
• Attempt to gain unauthorized access to the platform or other user accounts
• Disrupt or interfere with platform functionality
• Upload malicious code or harmful content

10. Reporting Concerns
Members are encouraged to report any concerns regarding:
• Violations of this Agreement
• Unprofessional or harmful behavior
• Privacy or confidentiality risks

11. Enforcement and Termination
The Collaborative reserves the right to:
• Remove content that violates this Agreement
• Suspend or terminate membership for violations
• Restrict access without refund in cases of serious misconduct

12. Modifications
This Agreement may be updated periodically. Continued use of the platform constitutes acceptance of any updates.

13. Acknowledgment
By joining the National Perinatal Loss Collaborative, you acknowledge that you have read, understood, and agree to abide by this Member Agreement and Code of Conduct.
